Phynd 360 provider data platform used by UNC to ensure higher-quality care via efficient roster and location management.
Phynd congratulates the UNC Senior Alliance, the ACO of UNC Health Care, which achieved the #1 rank nationally in clinical quality for all Pay-for-Performance Next Generation ACOs. The UNC Health Alliance topped 33 other ACOs, receiving a clinical quality score of 98.64 percent from the Centers for Medicare & Medicaid Services (CMS) for Performance Year Three of the Next Generation ACO Model. The results were released by CMS on January 10, 2020.
The UNC Senior Alliance relies on the Phynd 360 provider information management platform for maintaining provider profiles, including roster management and reporting, as well as location management. Phynd helps the UNC team maintain a profile for each location, including providers that work there, available services, and other information used to assist the coordination of and referrals for care.
The Phynd 360 is a key ingredient in UNC’s drive to achieve high provider data integrity, which UNC’s leaders see as essential for value-based care delivery. Phynd helps ensure that UNC Senior Alliance patients see the right provider, with the right expertise, in the right location, participating in the right ACO, network or health plan, to fully meet their care needs.

The UNC Senior Alliance is UNC Health Care’s statewide, physician-led accountable care organization, transforming healthcare delivery and payment models on behalf of over 3,000 physicians. The Senior Alliance is comprised of UNC Health Care hospitals and employed providers, as well as independent, community-based providers.
