Southwestern Health Resources Recognized as Number One in Medicare Savings for the 2nd Straight Year
DALLAS, TX, February 28, 2020 – Phynd congratulates Southwestern Health Resources (SWHR), Dallas, TX, for being ranked number one in Medicare savings for the second year in row, as recently reported by The Centers for Medicare & Medicaid Services (CMS).
SWHR, an Accountable Care Organization (ACO) model formed via the joint partnership between UT Southwestern Medical Center and Texas Health Resources has been participating in the CMS Next Generation ACO Model since 2017.
The Next Generation ACO Model is an initiative for ACOs that have experience in coordinating care for populations of patients, with the goal to “test whether strong financial incentives, coupled with tools to support better patient engagement and care management, can improve health outcomes and lower expenditures.” ACOs that participate are evaluated on their ability to deliver better care, better overall population health, while lowering expenditures. It is Medicare’s highest risk model.
SWHR manages the care for over 650,000 patients, with their network encompassing 29 hospital locations and 5,000+ providers. They are the largest ACO in North Texas and the fourth largest in the country. Since joining the model in 2017, SWHR has saved over $67 million with over $37 million in savings coming from 2018.
Prior to their participation in the Next Generation Model, SWHR also experienced great success in CMS’s Medicare Shared Savings Program (MSSP) saving over $73 million. A recent article on Business Wire provides greater detail on the history and success of SWHR.
“Being ranked number one in the nation for the second year in a row further demonstrates the success of the physician-driven strategies SWHR has designed to build a stronger system of care,” said Andrew Ziskind, MD, Senior Executive Officer of SWHR.
In Phynd’s view, the success of any Accountable Care network is dependent in part on reliable, up-to-date provider data to deliver high-quality care. In a high-risk environment, patients must be matched to the right provider, that can provide the right service, at a convenient time and place, every time.
Phynd 360 has been deployed as the provider data management platform at SWHR. Phynd helps ensure that provider data – people, places, services – are up to date. SWHR utilizes the Phynd platform to maintain provider profiles for each location, including all providers that work there, services available, and other information critical to care coordination. Phynd also enables easy-to-use roster and location management, as well as provider reporting.
Phynd serves as a central hub for all provider data, and enables optimization of EHR, Marketing and Claims systems via platform tools for provider enrollment, management, outreach and search across the enterprise.
Congratulations again, to SWHR on their achievements and their continued success!
